Transition Implementation Tools

This section provides information to help with putting an effective transition process into place in your school or community. Included are how-to guides and checklists.

Career and Technical Skills Achievement Profile is a model Profile for students with disabilities who are pursuing an Individualized Education Program (IEP) Diploma. The Profile documents a student's attainment of career and technical knowledge and skills; work-related skills and certification or licenses and was jointly developed by the Offices of Curriculum and Instructional Support and Vocational and Educational Services for Individuals with Disabilities (VESID). (Please note: the profile should now be used for students aged 15-21.)

Effective Practices: Transition Planning and Services provides information on effective practices for students with disabilities engaged in the transition process.

Transition Tool Box provides a wide range of resources that the Transition Coordination Sites, VESID’s primary providers of technical assistance around transition issues, have found most helpful in assisting students, families, schools and community agencies in developing and implementing effective transition planning and services.
